Escala de Cleveland vs. Criterios de Roma IV para estreñimiento Crónico en Adultos: Estudio Piloto
Resumen
Antecedentes y objetivos: El estreñimiento crónico (EC) es uno de los principales trastornos gastrointestinales a nivel mundial y para su diagnóstico se emplean los Criterios de Roma IV (CR-IV), aunque no miden la severidad del síntoma como la Escala de Cleveland de Estreñimiento (ECE). El objetivo de este trabajo es comparar la ECE y los CR-IV. Métodos: Es un estudio piloto prospectivo en pacientes del Hospital Universitario de Puebla quienes refieren padecer EC. La ECE es una herramienta que identifica el EC y cuantifica la severidad; mientras que los CR-IV para EC son la prueba más utilizada para su diagnóstico. Resultados: Se incluyeron a 13 pacientes, identificando con caso positivo 7.7% con ambas pruebas, 15.4% con ECE y 23.1% con los criterios de Roma IV. Observando un grado de acuerdo bajo (κ = 0.030) y simetría en la identificación de diagnósticos entre los dos instrumentos. Conclusiones: Se encontró que estos instrumentos evalúan aspectos complementarios del EC, por lo que se señala la importancia de implementar el análisis de la severidad del EC al considerar un impacto más profundo en la presentación clínica de los pacientes para su evaluación y seguimiento clínico.
