Manejo del Paciente Inmunosuprimido Sometido a Cirugía: Una Revisión Narrativa

Resumen

Introducción. El paciente inmunosuprimido constituye una población heterogénea en expansión, dada la prescripción creciente de fármacos modificadores de la enfermedad, terapias biológicas, inhibidores de cinasas Janus (JAKi), agentes antineoplásicos y regímenes de inmunosupresión post-trasplante. Su manejo perioperatorio plantea una tensión clínica fundamental entre el riesgo de infección y dehiscencia de herida —si la inmunosupresión se mantiene— y el riesgo de exacerbación de la enfermedad de base con potenciales complicaciones organoespecíficas o rechazo del injerto— si se interrumpe. Objetivo. Sintetizar críticamente la evidencia contemporánea sobre la evaluación preoperatoria, el manejo farmacológico perioperatorio según clase terapéutica, la profilaxis de complicaciones y el seguimiento posoperatorio del paciente inmunosuprimido sometido a cirugía electiva o urgente, con especial atención a la aplicabilidad en escenarios de medicina familiar y atención primaria. Métodos. Revisión narrativa estructurada con búsqueda no sistemática en PubMed, EMBASE, Cochrane Library y Epistemonikos hasta marzo de 2026, complementada con guías de práctica clínica de las sociedades ACR/AAHKS, EULAR, BSR, ESC, ISHLT y la Association of Anaesthetists. Resultados. La estratificación individualizada del riesgo debe integrar variables del huésped, del procedimiento y farmacológicas. Los DMARDs convencionales (metotrexato, hidroxicloroquina, sulfasalazina, leflunomida) habitualmente se continúan; los biológicos y los inhibidores de proteína quinasa se interrumpen y la cirugía se programa al final del ciclo de dosificación; los JAKi se suspenden tres días antes del procedimiento. Los glucocorticoides crónicos requieren evaluación del eje hipotálamo-hipófiso-suprarrenal y, según el riesgo, suplementación con dosis de estrés. En trasplante de órgano sólido, la inmunosupresión de mantenimiento generalmente no se interrumpe, sino que se optimiza farmacocinéticamente. Conclusiones. El manejo perioperatorio del paciente inmunosuprimido exige un abordaje multidisciplinar individualizado, integrando perspectiva del especialista tratante, anestesiólogo, cirujano y médico de atención primaria. La evidencia sigue siendo predominantemente observacional, con áreas de incertidumbre relevantes en agentes biológicos de introducción reciente, JAKi y poblaciones complejas.
