Revisión: Inmunología de la Histoplasmosis

Resumen

La histoplasmosis es una micosis crónica asociada principalmente con manifestaciones pulmonares, aunque puede comprometer otros órganos, particularmente en el sistema reticuloendotelial. La patología se asocia con miembros del complejo Histoplasma capsulatum, hongos dimórficos presentes en suelo, comúnmente asociados con ambientes ricos en excretas de palomas y guano de murciélago. En el ambiente, el hongo forma tanto macro- conidias como micro- conidias, siendo más fácil para estas últimas alcanzar alveolos pulmonares. La vía de ingreso a la célula se realiza principalmente por las vías tradicionales de fagocitosis. Los procesos de inmunidad innata reaccionan como primera línea de defensa ante la infección. Sin embargo, el microorganismo cuenta con mecanismos de evasión que pueden dificultar el reconocimiento celular. En términos de inmunidad adaptativa, las citoquinas de la respuesta Th1 y Th17 promueven la resolución del cuadro infeccioso, en tanto que las propias de la respuesta Th2 y Treg exacerban la histoplasmosis. Pacientes inmunocomprometidos presentan una mayor afectación y diseminación, dadas las características de debilitamiento inmune. Como estrategias de defensa ante el cuadro, se exploran alternativas de vacunación y se plantean modelos teóricos como la deprivación de metales traza. Sin embargo, se requiere una investigación más extensiva que permita plantear metodologías adecuadas para contrarrestar la patología.
