Revisión bibliográfica sobre definición, criterios diagnósticos y tratamiento de obesos metabólicamente sanos

Resumen

Un grupo especial de personas con obesidad o sobrepeso que carecen o no presentan alteraciones metabólicas se las ha descrito o denominado como obesos metabólicamente saludables. Sin embargo, los individuos pueden estar libre de complicaciones relacionada con la obesidad, pero es probable que no estén completamente exentos de desarrollar algún trastorno cardiometabolico. La obesidad se ve asociada a un grupo de trastornos que presentan una relación con la diabetes mellitus tipo 2, hipertensión y la dislipidemia que en conjunto se denomina síndrome metabólico. La obesidad se ha incrementado en adultos y drásticamente en niños y adolescentes cuya prevalencia puede alcanzar un 20% para 2025. La prevalencia de obesos metabólicamente sanos va del 6% al 60% o 75%, y varía según factores sociodemográficos, como el origen étnico y la edad, por lo tanto, surge la importancia de caracterizar a los individuos metabólicamente sanos. Existen criterios que se los van a obtener en base a un entorno clínico ambulatorio sin embargo otros necesitan pruebas más sofisticadas. Existen varios criterios para el diagnóstico como: Glucosa en ayunas, colesterol HDL, prueba de tolerancia a la glucosa oral, presión arterial. Los factores del estilo de vida se ven influenciados en estos individuos, es importante mencionar sobre el riesgo cardiovascular que pueden llegar a presentar. El tratamiento de primera línea recomendado para obesos metabólicamente sanos es la pérdida de peso, actualmente se habla de la cirugía bariátrica como estrategia de pérdida de peso efectiva.
