Reporte de Caso: Síndrome Postcolecistectomía Secundario a Remanente de Muñón Vesicular
Resumen
La colelitiasis es una entidad frecuente a nivel mundial, en México, alrededor del 20% de los hombres y entre el 40% y el 50% de las mujeres sufren de colelitiasis1, como complicación de la litiasis vesicular se puede presentar un cuadro de colecistitis aguda, requiriendo para esta condición patología la extirpación de la vesícula biliar, la colecistectomía considera el tratamiento de elección para el manejo de los pacientes con colecistitis3, a pesar de ser un procedimiento generalmente seguro existe complicaciones reportadas en la bibliografía, una de ellas es el síndrome postcolecistectomía, que se define como la recurrencia de síntomas similares a los experimentados antes de la colecistectomía8, la incidencia reportada en la bibliografía es bastante variable, algunos autores reportan que se presenta hasta en el 47% de los pacientes colecistectomizados. Existen múltiples causas de este síndrome, una de ellas es la existencia de remanentes vesiculares o conductos císticos largos. Se reporta el caso de una paciente colecistectomizada, quien 4 años posteriores a su intervención quirúrgica presenta cuadro de dolor abdominal en hipocondrio derecho, se inicia abordaje con laboratoriales, ecografía y tomografía, integrandose diagnóstico de colecistitis de remanente vesicular.
