Revisión Sistemática sobre Estrategias de Prevención y Manejo de la Insuficiencia Cardíaca en Poblaciones Socioeconómicamente Desfavorecidas: Enfoque Aplicado al Contexto Ecuatoriano
Resumen
La insuficiencia cardíaca se presenta como una de las principales razones de enfermedad y muerte a nivel global, impactando especialmente a las poblaciones en condiciones de vulnerabilidad económica y social 1. Esta revisión sistemática, desarrollada conforme a los lineamientos PRISMA 2020, tuvo como objetivo identificar y analizar las estrategias más efectivas de prevención y manejo de la enfermedad, con énfasis en su aplicabilidad al contexto ecuatoriano. Se realizó una búsqueda en las bases de datos PubMed, Scielo, ScienceDirect, Google Scholar y Cochrane, seleccionando 80 estudios para análisis cualitativo y 18 para análisis cuantitativo publicados entre 2019 y 2024. Los hallazgos evidencian que la prevención primaria, mediante el control de factores de riesgo y la educación sanitaria, constituye una medida costo-efectiva2. Asimismo, la terapia farmacológica cuádruple reduce de forma significativa la mortalidad y las hospitalizaciones; sin embargo, su acceso sigue siendo limitado en sistemas de salud con recursos restringidos3. Tecnologías emergentes como la telemedicina ofrecen nuevas oportunidades, aunque aún enfrentan barreras relacionadas con infraestructura y equidad4. En conjunto, los resultados subrayan la necesidad de políticas públicas que fortalezcan la atención primaria, garanticen el acceso universal a medicamentos esenciales y adapten las guías internacionales a la realidad ecuatoriana5.
