Eficacia y Seguridad del Dupulimab en Dermatitis Atópica Moderada a Severa en Niños. Revisión Sistemática
Resumen
La dermatitis atópica (DA) es una enfermedad inflamatoria crónica y pruriginosa frecuente en la infancia, con impacto significativo en la calidad de vida. Los tratamiento tópicos son la primera línea, mientras que los sistémicos se reservan para casos severos. Dupilumab, un anticuerpo monoclonal que bloquea IL – 4 e IL – 13, ha demostrado eficacia en DA moderada a grave. Se revisaron ensayos clínicos controlados, doble ciego, en pacientes pediátricos de 6 meses a 11 años (Siegfried et al., 2023; Paller et al., 2020). En Siegfried et al., 162 pacientes <6 años tratados con dupilumab + corticoesteroides tópicos mostraron mejoras significativas en EASI y signos clínicos desde la semana 2 hasta la 16 (p<0.0001). En Paller et al., 367 niños de 6 – 11 años alcanzaron mayor IGA 0 o 1 (29.5-32.8% vs 11.4%) y EASI – 75 (67 – 69.7% vs 26.8%), con reducción de prurito desde la semana 4 (p≤0.0001). En conclusión, dupilumab combinado con corticoesteroides tópicos es seguro y eficaz en DA pediátrica, proporcionando mejoras tempranas y sostenidas en severidad y prurito.
