Evolución de una herida crónica en miembro inferior con tratamiento farmacológico y curaciones locales
Resumen
Las heridas crónicas de los miembros inferiores representan un reto clínico por su evolución prolongada y por la interacción de factores locales y sistémicos que pueden retrasar la reparación tisular. Se presenta el caso de una mujer de 66 años, sin antecedentes de diabetes mellitus ni hipertensión arterial, que desarrolló una herida traumática en la región distal de un miembro inferior después de golpearse con un mueble de su domicilio. En la valoración inicial presentó dolor y edema depresible, con retorno aproximado de cinco segundos después de la digitopresión. Los estudios basales se realizaron el 2 de diciembre de 2024 y el seguimiento se documentó hasta julio de 2026. El manejo incluyó curaciones con Microdacyn® spray cada 12 horas durante los primeros tres meses y una vez al día posteriormente, aplicación de Recoverón® N ungüento, furosemida 40 mg cada 24 horas por el edema, diosmina/hesperidina y vitamina E 40 UI. Las imágenes mostraron disminución progresiva de la superficie abierta y cierre aparente, con hiperpigmentación residual. La evolución ocurrió bajo un tratamiento combinado, por lo que no puede atribuirse a una sola intervención.
