Prurito Anal: la Importancia de la Biopsia de Piel
Resumen
Antecedentes y objetivos: El prurito anal es una condición frecuente caracterizada por picor perianal, con etiología multifactorial y alta variabilidad clínica. El objetivo fue identificar la relación entre las características clínicas y los hallazgos histopatológicos en pacientes con prurito anal. Métodos: Estudio prospectivo, transversal y observacional en pacientes del Hospital Universitario de Puebla. Se incluyeron pacientes con diagnóstico clínico de prurito anal, a quienes se realizó biopsia perianal con punch de 4 mm. Se analizaron variables sociodemográficas, clínicas, hábitos higiénico-dietéticos y hallazgos histopatológicos. Resultados: Se incluyeron 26 pacientes, con predominio femenino (69.2%) y edad de 24 a 81 años. El prurito se presentó principalmente en relación con la evacuación y en horario nocturno, con evolución cercana a un año. La clasificación tipo II del Washington Hospital Center fue la más frecuente (73%). Se identificaron 18 diagnósticos histopatológicos distintos, siendo el liquen simple el más común. Conclusiones: El prurito anal presenta alta heterogeneidad clínica e histopatológica, sin relación lineal entre ambas. La evaluación clínica aislada no permite predecir los hallazgos tisulares, por lo que la biopsia perianal es útil en casos seleccionados.
