Trauma Vesical Intraperitoneal Asociado a Fractura Inestable del Anillo Pélvico: Manejo Quirúrgico y Lecciones
Resumen
Las fracturas inestables del anillo pélvico por trauma de alta energía se asocian frecuentemente con lesiones viscerales, incluida la lesión vesical. Se presenta el caso de un varón de 21 años con politraumatismo por accidente en motocicleta, que ingresó en choque hipovolémico, hematuria macroscópica e irritación peritoneal. La tomografía evidenció fractura de anillo pélvico AO 61C1.3. Se realizó laparotomía exploradora urgente, confirmándose rotura vesical intraperitoneal grado IV (AAST), tratada mediante reparación en dos planos y fijación externa supracetabular. La evolución fue favorable, con control hemodinámico y preservación funcional urinaria. El caso enfatiza la importancia de la sospecha clínica y el manejo quirúrgico oportuno en pacientes con hematuria macroscópica y fractura pélvica inestable.
