Eficacia, Adherencia y Barreras en el Uso de PrEP y PEP para la Prevención del VIH en Poblaciones de Alto Riesgo: Una Revisión Sistemática
Resumen
La profilaxis preexposición (PrEP) y la profilaxis posexposición (PEP) son intervenciones fundamentales en la prevención biomédica del VIH, especialmente en poblaciones con alto riesgo de exposición. Esta revisión sistemática analiza la evidencia disponible sobre la eficacia clínica, la adherencia terapéutica y las barreras de acceso asociadas al uso de PrEP y PEP entre 2014 y 2024. Se incluyeron 25 estudios, que reportaron una eficacia de hasta 99 % con PrEP y alta efectividad con PEP cuando se inicia dentro de las 72 horas posteriores a la exposición. La adherencia mostró variabilidad, influenciada por factores individuales, contextuales y estructurales. Entre las principales barreras se identificaron el estigma, la baja percepción de riesgo y las limitaciones en el acceso a servicios de salud. Se concluye que, para maximizar el impacto de estas estrategias, es necesario implementar modelos integrales que combinen educación, acompañamiento clínico, uso de tecnologías móviles y políticas inclusivas.
